Periodontal Maintenance Cleaning

The key to preventing periodontal disease is early diagnosis and treatment. This entails periodontal maintenance after your initial therapy of scaling and root planing. If not cleaned out every 3 to 4 months, bacterial biofilms can grow and predominate.

Active Therapy
Scaling and root planing removes disease below the gum line.
3-Month Visit
First maintenance cleaning to assess healing and reduce bacterial load.
Ongoing Monitoring
Every 3–4 months until gum health is stable and no rebound occurs.
Stable Schedule
Visits continue at the interval that keeps your gums healthy long-term.

Your periodontal maintenance visit will follow three months after your initial therapy or scaling root planing procedure. During this visit, we will probe along the gum line and check for improved measurements of gum inflammation. We will maintain these visits until we are sure there will be no rebound to the original state of inflammation and bacterial load, and we have confidence that you will remain stable. If periodontal visits are not followed through, there is a possibility for relapse, and scaling and root planing will be necessary a second time.

Why the 3 to 4 Month Interval Matters

Bacterial biofilm reforms along and beneath the gumline within weeks of a thorough cleaning. At a standard twice-yearly cleaning interval, those biofilms have months to mature, calcify, and re-trigger the inflammatory cycle that drives bone loss. Periodontal maintenance at 3 to 4 month intervals disrupts this cycle consistently enough to keep bacterial levels below the threshold that causes active tissue damage. Research consistently shows that patients who adhere to a maintenance schedule after SRP maintain better pocket depths and experience significantly less disease progression than those who do not. Skipping visits does not just delay progress, it can allow the disease to rebound to the point where additional scaling and root planing is needed again.

What Happens at Your Maintenance Visit

Each periodontal maintenance appointment is more thorough than a standard cleaning. We probe around every tooth to track pocket depths and compare them against your baseline measurements, looking for any signs of inflammation or rebound. We then clean above and below the gumline, targeting the areas most prone to biofilm accumulation in your specific case. We review your home care routine and refine it based on what we observe, because certain areas consistently retain more plaque for different patients and your technique can be adjusted accordingly. As the interval between visits stabilizes and pocket depths remain healthy, we will discuss whether you can transition to a less frequent schedule. Staying consistent with your maintenance schedule is one of the most important things you can do for your gum health and your smile long-term.

How Periodontal Maintenance Differs From a Regular Cleaning

A standard prophylaxis cleaning — the kind most patients receive twice a year — is designed for mouths with healthy gum tissue and shallow pocket depths. Periodontal maintenance is a different procedure entirely. It involves probing every tooth at multiple points to track pocket depth changes, cleaning below the gumline into areas that a routine cleaning does not reach, and assessing bone levels and tissue health against your prior baseline. Your hygienist and Dr. Coambs review the data from each visit to determine whether your gum disease is stable, improving, or showing signs of rebound. That clinical comparison is what makes periodontal maintenance protective rather than simply routine — it catches changes early, when intervention is still straightforward.

Your Gum Health Is Worth Staying On Top Of.

Regular periodontal maintenance keeps the progress you’ve made — and protects everything else in your mouth. Dr. Coambs recommends staying on schedule.
