Once you have requested an appointment with Imagine Dentistry, our team will contact you to verify your insurance information and schedule a date for your consultation. You will also receive an email with the new registration forms so you can fill them out in the comfort of your own home and save time at our office.
If you have dental insurance, filling out this information will allow us to verify your benefits prior to your first appointment. Forms submitted with incorrect information (i.e. medical insurance vs. dental insurance information), or forms submitted at the last minute, may not allow us to confirm your insurance benefits. In this case, you may be required to pay for your appointment in full at the time of service, and then submit your own claims for reimbursement.
